Lodi and Tokay High School scheduling for 2017-2018 9th Graders will be during the school day at Millswood during the week of 3/13/17.

Tokay High School will be offering $20.00 Sports Physicals in the school gym for girls on May 23rd and boys on May 24th, both starting at 4:00, which will be administered by the Lodi Health staff.  The physical will be good for one year.  Lodi Health requires that the front page of the physical be filled out by a parent before the examination, so I have attached a copy of it for you.  It is not to be used to take to their own physician, but you can down load a copy of the Tokay physical form on the Tokay website, under Athletics.  You will also see all the other forms required to participate in sports, which is only required yearly, not for each season.
